Cauda equina syndrome is a relatively uncommon disorder affecting the bundle of nerve roots at the lower end of the spinal cord, which is known as the cauda equina. The term, loosely translated, means the “horse's tail." Cauda equina syndrome typically presents with severe lower back pain, sciatica and/or weakness in one or both legs, and possibly loss of bladder or bowel function. This syndrome is usually caused by a disk herniation or other issue that compresses the cauda equina nerve bundle.
Although the symptoms of cauda equina syndrome may appear in other disorders, it's critical that medical professionals consider the possibility cauda equina syndrome. This syndrome is typically diagnosed with an MRI. If not diagnosed promptly, nerve damage and deficits can become permanent, leading to chronic pain, permanent weakness and loss of function in the legs, and permanent sexual, bowel, and bladder dysfunction. Cauda equina is a medical emergency.
Doctors, especially those who work in hospital emergency rooms where acute cases of back pain often present, should be aware of the significance of the constellation of cauda equina symptoms and be prepared to order the appropriate tests and decompression surgery if needed. If cauda equina is suspected, a neurosurgical consult is the standard of care and must be done without delay. Failure to do so may constitute medical malpractice.
